Following are forms that Clubs and Participants may need to use from time to time to provide information to Football St George.

DISCIPLINARY & INCIDENT FORMS

The following forms can be found on the FSG Disciplinary Page:

  • Non SGFRA Referee Report (Winter Football)
  • Send-off Report (Summer Football)
  • Incident Report (Winter & Summer Football)
  • Challenge Form (Winter & Summer Football)
  • Appeal Form (Winter & Summer Football)

CLUB PLAYER FORMS

The Player Re-Grade Request Form is to be completed by a Club when they wish to re-grade a player after the player has registered, and up until 30 June in the season.

The Playing Above Age Request Form is to be completed where a player seeks to play in an age grade two years above their age (i.e. a 12 year old wanting to play in a U15 competition).

In addition to completing the form, the player will need to be assessed by the FSG Technical Director prior to a decision being made about the ability of the player to play above their age.

CLUB COMPETITION FORMS

The Inter-Association Trial Match Form needs to be completed by clubs who wish to play trial matches against clubs from a different Association

Clubs are required to complete the Fixture Alteration Request form to request changes to the match schedule after it is released.

Clubs are required to complete the Forfeit Form when they are aware ahead of match that they are unable to field a team. This form informs FSG, the opposing club, and the referees.

CLUB PROMOTION FORMS

The Club Advertisement Form provides clubs an opportunity to submit stories they would like promoted on FSG social media channels such as registration offers, events, coach and player vacancies.

FACILITY FORMS

The Grounds and Maintenance Registers enable Clubs to report areas of concern at the fields they use for training and matches.
